Care Coordinator (ICC), Hamilton County

We are seeking a Part-time Individualized Care Coordinator (ICC) / Case Coordinator to work in the Home and Community Based Services Waiver Program, providing case management services to families of children with Serious Emotional Disturbance covering Montgomery and Hamilton Counties, based out of the Gloversville office.
The families are identified by the county as needing high levels of services to maintain their children in the community. This position requires flexibility of schedule and also requires that the employee is comfortable working with families in their homes.
Specific duties include the following:
+ Complete an initial assessment of client strengths and needs
+ Development of written service plans, engaging a team consensus approach
+ Assist family in enrolling child in Medicaid and / or Medicaid Managed Care
+ Facilitate development of initial service plan, cost out budget based on service plan
+ Coordinate planned care
+ Monitor service delivery and supports, budget development and review
+ Receive, review, and monitor original progress notes submitted by subcontractors
+ Deliver clinical services as needed
+ Act as primary coordinator to assess needs, develop service plans, advocate, monitor, plan for discharge, and provide consultation
+ Complete all record keeping and reporting requirements associated with program
+ Masters degree in human services field and one (1) years experience in providing direct services to or in linking children with serious emotional disturbance to services. OR
+ Bachelor s degree in human services field and two (2) years experience in providing direct services to or in linking children with serious emotional disturbance to services. OR
+ NYS Teacher s Certificate Experience and two (2) years experience in providing direct services to or in linking children with serious emotional disturbance to services to a broad array of services.
+ Must have valid NYS drivers license and access to a reliable vehicle.
This position does not include the scope of practice to qualify for an LCSW or LMHC in New York State.
